Required Forms
Before participating in Career Explorations, all registrants must complete and submit the following forms in their entirety. Anyone who does not complete the forms will be unable to attend. Form signatures can be digital or handwritten.
CX Registration & Medical Release Form (PDF)
Forms must be received no later than two weeks prior to the event date. Upload them via our secured form submitter. Make sure to select the campus you’re visiting under the “Campus Applied To” drop-down.
