Before enrolling at Johnson & Wales University, all new students must submit completed health forms by no later than the Immunization & Health History Requirements Deadline. We encourage you to submit forms as early as possible because until the required documentation/records have been submitted and verified, students will be unable to receive a course schedule, register for classes, or select on-campus housing. If you have questions during this process, please contact us.

Other Forms

Students who need or require immunization exemptions based on religious or medical reasons should complete an applicable form (provided below) and upload it to the health portal. Providence Campus students who are in the process of acquiring their vaccinations should complete the Temporary Immunization Exemption Certificate to document scheduled appointments to receive immunizations. 

Additional Instructions for Student-Athletes

In addition to the Immunization & Health History Form, student athletes must complete additional steps. Please make sure to verify your health insurance coverage for athletic participation. It is recommended that you hand in your completed Athletic Health Forms to a member of the athletic training staff when you arrive on campus. For more information, please visit:

Optional State Immunization Records Import

For a service fee of $20, request an automatic import of your immunization records directly from your state registry — no health provider visit or paperwork needed. To proceed, log in to the health portal and go to the Immunizations tab. Note: This service is NOT available for all states. The Immunizations tab in the health portal will provide details on participating states and eligibility. Again, this service is optional and you may follow the standard instructions provided on this web page. 

Step 2: Upload Completed Forms & Documents

Students must log in to the health portal using their JWU Wildcat Email login and password. Once you enter the portal, upload forms, manually enter your vaccine dates, and complete your emergency contact information. 

Log in to JWU's PyraMed Student Health Portal

Note: Students can access the health portal anytime via jwuLink (must be logged in using the JWU Wildcat Email login and password). Type "health portal" in the Search bar or go to the Tools menu > Grades & Personal Info category.

Upload Instructions

  • Before uploading, review all forms carefully and make sure they are fully completed, signed, and dated.
  • Acceptable file upload types are JPEG, TIFF, PNG, or PDF only (convert files as needed).
  • Each file must be less than 6MB (compress file sizes or lower resolutions as needed).
  • After you log in to the health portal, select the Document Upload tab. Use the dropdown to select which form you are uploading. Then, choose files from your phone or computer and SAVE. Wait for a pop-up confirmation to ensure your upload is successful. Repeat this process for each file that you are uploading.
  • You will see a "pending" message until your uploads are verified by JWU Health Services. Please allow 2-3 business days for review (students are reviewed individually in the order records were received).
  • If information is missing, a message will be sent to your JWU Wildcat Email address. Please check your inbox regularly.
  • If you are having upload problems, please contact us. We can provide help or offer other ways for you to submit your documents.

Important to Review: JWU Student Health Insurance Plan

Undergraduate day students, both domestic and international, all students enrolled in the Physician Assistant and Counseling graduate student programs, and all international graduate/doctoral students attending Johnson & Wales University are required to have health insurance coverage that is accepted in the United States. For an affordable fee and year-round coverage, Johnson & Wales University offers students who meet these criteria, comprehensive medical insurance with the Student Health Insurance Plan (SHIP). The SHIP is serviced by University Health Plans (UHP) and underwritten by United HealthCare. Policy information and a benefits overview is available on the SHIP websiteStudents are automatically enrolled in the SHIP. Students who wish to utilize an alternate and active health insurance policy are required to waive automatic enrollment into the SHIP by the stated deadline. Carefully review both the brochure and the FAQs before deciding whether to waive the SHIP. We encourage you to ensure that your health insurance coverage meets the following criteria:

  • The claims administrator of the plan is based in the U.S. and has a U.S. telephone number and address.
  • The plan provides both emergency and non-emergency health care, hospitalization and mental health benefits within your local campus area.
  • The plan provides inpatient and outpatient mental health care and substance abuse benefits.
  • The plan provides coverage for prescription medication.
  • The lifetime benefit is unlimited.
  • Out-of-state Medicaid health plans, state sponsored Children’s Health Insurance Plans, HMOs, and Kaiser Permanente may not cover non-emergency care in your local campus area. Please review your specific policy information before deciding to waive the Johnson & Wales Student Health Insurance Policy offered by United HealthCare.

Waive or Keep JWU Health Insurance

You are not required to have the university sponsored health insurance in order to utilize JWU Health Services or JWU Counseling Services on campus. Regardless of insurance carrier, all Johnson & Wales students are able to utilize on-campus Health and Counseling Services at no additional charge.
