Review & Submit Health Forms

Before enrolling at Johnson & Wales University, all students must submit their completed health forms. If you have questions, please contact your campus Health Services department using the information listed below.

2. Upload Your Forms

Once you have completed your health forms, open the PyraMed Student Patient Portal in jwuLink (Tools > Health Services) or go directly to https://jwu.studenthealthportal.com and log in with your JWU credentials. Once you enter the Patient Portal you can upload your forms, manually enter your vaccine dates, and complete your emergency contact information.

Make sure to verify your insurance coverage for athletic participation. It is recommended that you hand in your completed Athletic Health Forms to a member of the athletic training staff when you arrive on campus.


Important Information about Health Insurance for JWU Students

Undergraduate day students, both domestic and international, all students enrolled in the Physician Assistant and Counseling graduate student programs, and all international graduate/doctoral students attending Johnson & Wales University are required to have health insurance coverage that is accepted in the United States. For an affordable fee and year-round coverage, Johnson & Wales University offers students who meet these criteria, comprehensive medical insurance with the Student Health Insurance Plan (SHIP).

Regardless of insurance carrier, all Johnson & Wales students are able to utilize on-campus Health and Counseling Services at no additional charge. Explore the health and counseling services available to you while on campus.

The Student Health Insurance Plan is serviced by University Health Plans (UHP) and underwritten by United HealthCare. Policy information and a benefits overview is available on the SHIP website.

Students who wish to utilize an alternate and active health insurance policy are required to waive automatic enrollment into the Johnson & Wales Student Health Insurance Policy by the stated deadline. Carefully review both the brochure and the FAQs before deciding whether to waive SHIP on the SHIP website.

We encourage you to ensure that your health insurance coverage meets the following criteria:

  • The claims administrator of the plan is based in the U.S. and has a U.S. telephone number and address.
  • The plan provides both emergency and non-emergency health care, hospitalization and mental health benefits within your local campus area.
  • The plan provides inpatient and outpatient mental health care and substance abuse benefits.
  • The plan provides coverage for prescription medication.
  • The lifetime benefit is unlimited.
  • Out-of-state Medicaid health plans, state sponsored Children’s Health Insurance Plans, HMOs, and Kaiser Permanente may not cover non-emergency care in your local campus area. Please review your specific policy information before deciding to waive the Johnson & Wales Student Health Insurance Policy offered by United HealthCare.

You are not required to have the university sponsored health insurance in order to utilize JWU Health Services or JWU Counseling Services on campus.
